Assistance Applications for Online intake however remain here. If you are not able to complete an online application, please leave us a voicemail at 808-949-2922 or 800-882-1057 with your name and best contact information and a member of our intake team will be in contact. Starting Monday, September 7, we will also accept walk-ins from 8:30am - 4:30pm.
