Choosing your treatment and care in advance isn’t just a good idea, it’s your right. An advance directive is a legal document that incorporates all the functions of a:

• Healthcare proxy
• Substitute decision-maker
• Attorney-in-fact
• Do not resuscitate (DNR) order

When an advance directive is correctly implemented and executed, it:
